Commercial Plans

We are contracted with several carriers but we recommend checking with your plan to find in-network providers.

Out of Network

We are NOT preferred providers with the following carriers and your costs will be higher with these plans.

  • Cigna/Evernorth

  • United Healthcare

  • UMR

  • Aetna 

Medicare and Medicaid (OHP, Trillium, PacificSource Community Solutions)

Medicare and Medicare supplement plans cannot be billed as the providers have all opted out.  

There are no Medicaid providers in this group.

Please contact your Oregon Health Plan coordinated care organization or health plan for a list of providers.

Self Pay

We would be happy to discuss our discounted rates for those who are uninsured.
